How sample tags are serialized.
Pilot tags are stamped SAMPLE and registered to your establishment only. Each serial carries your establishment registration and its count — e.g. FF-SMP-<EST>-01/01 for a single kit, or 02/03 for the second of three on file. Entering any sample serial on the Verify-a-tag page returns: sample only · registered to your establishment · quantity issued. Sample tags cannot be activated for a family and are never valid in a custody record.

Kits are fulfilled through licensed funeral establishments. License numbers are verified against the state licensing registry before a kit ships.
